Registration packets are only available at our field due to requirements for forms to be on specific colors and/or card stock. The forms listed below are also available in the registration packets, but we have made them available for download so you can fill out prior to registering at the field.

Registration Forms

Please note we will provide all necessary registration forms as they must be on very specific paper. We have provided copies here for convenience so you know what information will be asked for. We do not recommend that you print these documents because they cannot be accepted unless they are printed on the correct paper

In addition to the Registration packet we will also need a copy of each athletes birth certificate and a recent wallet sized picture (within last 12 months).

We do have Notary's available at the field for registration packets free of charge.

FHSAA Physical Form
This form must be completed after May 1 and must have a doctor's stamp or it will not be accepted.

TCYFCC Waiver and Release of Liability
This form must be printed on yellow paper only or it will not be accepted.

TCYFCC Registration Hardstock
This form must be printed on Sand Stone Hardstock only.

Department of Parks General Release
This form must be printed on Green paper only.

Department of Parks and Recreation Medical Release
This form must be printed on Green paper only.
